For HSE patients (Medical Card /Doctor Visit Card holders) who are not yet registered with E&S Doctors, please fill in the form below to register.

To apply for your medical card, you must complete the HSE application using the link provided below. You can apply to the HSE online or by post.

If you choose to apply by post, please note: You will be required to visit the clinic in person with your physical MC1 form so it can be processed and signed by the doctor.
